You, the participant (or participant’s parent/legal guardian if participant is under 18 years old) authorize your (your child’s) full participation in the Crescendo All-State Masterclasses, produced by CrescendoDFW, held at Park Cities Baptist Church (PCBC), including related activities. You understand the activities are not without some inherent risk of injury. In consideration of your (your child’s) right to participate in this activity, you agree to release, waive, discharge, agree not to sue, and agree to hold harmless for any and all purposes, the Crescendo All-State Masterclasses, inclusive of CrescendoDFW, teachers, and volunteers, and PCBC, their pastors, elders, and officers, employees, agents, and volunteers (Releasees) from any and all liabilities, claims, or injuries, including death, that may be sustained while participating in this activity, including traveling to, from, and for the activity, or while on premises owned or controlled by Releasees, including injuries sustained as a result of the negligence of Releasees. You understand this release does not apply to injuries caused by intentional or grossly negligent conduct on the part of Releasees. You further agree to indemnify and hold harmless Releasees for any loss, liability, claim, or injury caused by you (your child) while participating in this activity, including traveling to, from, and for the activity, or while on premises owned or controlled by Releasees. You also give your permission for you (your child) to receive any emergency medical treatment by a healthcare professional, including emergency medical transportation, which may be required for injuries sustained by you (your child). You agree to indemnify and hold harmless Releasees for any cost incurred to treat you (your child), even if a Releasee has signed hospital documentation promising to pay for the treatment. State law requires you be informed of the following: (1) you are entitled to request to be informed about the information about yourself collected by use of this form (with a few exceptions as provided by law); (2) you are entitled to receive and review that information; and (3) you are entitled to have the information corrected at no charge to you.

 

You do hereby give Crescendo All-State Masterclasses, CrescendoDFW, and PCBC, its assigns, licensees, and legal representatives the irrevocable right to use your name (or any fictional name), picture, portrait, photograph, or video in all forms and media and in all manner, for advertising, trade, or in any other lawful purpose. You hereby forever waive any right to inspect or approve the finished product, including but not limited to, written copy and/or an image in print or on a web site that may be created in connection therewith. You understand that Crescendo All-State Masterclasses, CrescendoDFW, and PCBC cannot control the unauthorized use by persons other than themselves, of your name or image once such name or image is published. Any claim you may have concerning unauthorized publication of your name and image must be pursued by you against the unauthorized user. Crescendo All-State Masterclasses, CrescendoDFW, and PCBC disclaim any responsibility for such unauthorized use of your published name or image. You have had sufficient time to review and seek explanation of the provisions contained above, have carefully read and understand them, and agree to be bound by them. You voluntarily and irrevocably give your consent and agree to this Release and Waiver.

 

Students leaving the campus of PCBC for any reason without written permission from a parent or guardian will be dismissed from Crescendo All-State Masterclasses without refund. All personal property, including instruments, purses, including money are the responsibility of the owners at all times. Neither Crescendo All-State Masterclasses, CrescendoDFW, nor PCBC will take any responsibility for lost, stolen, or misplaced belongings. Pets, alcoholic beverages, knives, illegal drugs, smoking, gambling, explosives, and weapons of any kind are forbidden on the PCBC campus at all times. Violation of this policy will result in dismissal from the Crescendo All-State Masterclasses without refund as well as possible prosecution. Violation of the Crescendo All-State Masterclasses rules or dormitory regulations may result in the student's expulsion from the Crescendo All-State Masterclasses without refund. Parents will be notified, and arrangements will be made to send the student home at the parents’ or guardian’s expense. These terms and conditions apply between the Crescendo All-State Masterclasses and those persons registering for and participating in the Crescendo All-State Masterclasses events.
